When paying for a donation via automatically billing, we will charge your authorized credit card or electronic funds transfer (EFT) account on file in our Accounting Office. If you have not set up an automatic payment option with Ohev Shalom, please submit the appropriate form for credit/debit card deduction or checking account transfer.
For each donation, an acknowledgment card will be sent by the synagogue.
To complete this form, please provide your name, address, email and telephone. Select the amount you wish to donate and the fund to which you want to contribute. If you would like to make the donation in someone's honor or memory, please complete the last section. Press SUBMIT DONATION to send this form to Ohev Shalom. All funds, unless otherwise marked, have a minimum of $10 contribution.
